  • I am so happy you are here! I have been providing physical therapy in the DFW area since obtaining my doctorate in physical therapy in 2010 from Texas Woman’s University. The first 11 years of my career were at UT Southwestern’s outpatient clinic where I was able to treat a multitude of people with various musculoskeletal, orthopedic, cardiorespiratory and neurological disorders. The knowledge I obtained during this time was transformative in the healing care I am able provide to my patients today. No matter the obstacle big or small, I can help! I developed a strong love for treating those with neurological disorders and sought after extra certifications to become a a Board-Certified Neurologic Clinical Specialist. I worked closely with the movement disorder team at UT Southwestern and became known as the Parkinsons/movement disorder PT specialist. I worked with the movement disorder doctors on program development, research, neuromodulation networks pertaining to DBS candidacy and various trainings and presentations. I decided to leave UTSW in 2021 and start my own private pay concierge practice — Qualls Physical Therapy & Wellness, PLLC. Becoming “The Home Neuro PT” has allowed me to provide the necessary time and care specific to each person, without restrictions from insurance and with continuous care when needed. My goal is to focus on elite quality care and to never be someone’s limiting factor. My mission is to push boundaries and achieve the highest quality of life, rediscovering movement and to find joy for my patients after injury or diagnosis.

My Services

Physical Therapy Evaluation

60 - 75 minutes. 1-on-1 examination with your PT, to understand your needs and current functional levels. You will collaborate with your PT to set goals to meet YOUR needs.


Physical Therapy Treatment

50 - 60 minutes. 1-on-1 session with your PT, focused on enhancing movement and function both at home and in the community with a plan tailored specifically to YOU.


Wellness Care & Services

50 - 60 minutes. 1-on-1 session with your PT, to improve general health and wellbeing. Includes injury prevention training, general fitness, weight loss, and accountability programs, corrective posture exercise and top of the line supplements. 


Therapeutic stretching

45 minutes. 1-on-1 session with your PT for those that are feeling stiff. Examination of joint and soft tissue tightness prior to active, passive and or dynamic muscle energy stretching interventions. 

Therapy provided conveniently to your door. Service areas include Collin County, Dallas County, Denton County and Tarrant County.

Appointments take place in the same gym or clinic every time

F.A.Q.

  • Yes, if you are wanting Physical Therapy services provided, you must have a referral from a medical doctor with a diagnosis code.

  • No, you simply book a time with me.

  • With a referral for PT services, then we will begin with a PT session with you. If you do not have a PT referral, but you have the goals to be a healthier you with better mobility via professional guidance, then a wellness/training session may be better for you.

  • Treatment for each person will look different based on their unique needs. Some treatment techniques commonly used include: LSVT BIG treatment, PWR! Moves, boxing, gait retraining, neuromuscular re-education, balance training, agility training, postural retraining, yoga & pilates biased core work, aquatic therapy, autonomic nervous system training, general strengthening and conditioning via therapeutic exercise, vestibular rehabilitation, manual therapy, post-operative rehabilitation, sports rehabilitation, geriatric rehabilitation, modality therapy (including cupping, NMES, therapy percussion gun for trigger points, soft tissue massage, infrared light, heat/cold therapy, kinesiotaping, accutaping, lymphatic drainage techniques) and more.   • No, at this point all service is cash-based. You may pay by cash, credit card and/ or check. You can request a super bill to submit to your insurance in the case you have out of network coverage and are not a Medicare recipient.

  • QPT&W is not affiliated with Medicare.

  • Book one appointment for your initial visit. From there, we will determine how often you should return.

  • Physical Therapy and Wellness evaluations are $250 for a 60 - 75 minute session. Follow up appointments are $200 for 50 - 60 minute sessions. Shorter sessions are available dependent on location and cost will differ accordingly.

  • Absolutely! Currently at least 50% of my patients are more orthopedic cases and unfortunately most neurological cases will end up becoming orthopedic in nature at times. So you are in the best of hands by having me help you!

  • Sessions are held at the environment of your choosing. Typically in the home, but can also be at work, a park or at your gym. Location must be accessible and safe.

  • Yes, it is. Of course, there may be limitations to this but it is always an option to try out and if needed switch to in person consultation.

  • After consultation and history intake I can purchase them and mail/deliver to you in our session or you can set up a patient direct account and buy them directly.
